Description
NY is 60 year old, recently retired engineer with a history of hypertension, alcohol, and tobacco use who reports an increase in epigastric pain over the last 2-3 months. He reports occasional lightheadedness, fatigue, and black, “stinky” stools. Since retirement, he has been active with his buddies and has been taking NSAIDs daily for bilateral knee pain. Physical exam is significant for epigastric pain, tenderness, pale conjunctivae, tachycardia, positive guaiac, and early systolic murmur.
