Practicum Log Assignment Purpose

The purpose of the log is to document Practicum Project, Practicum Mentorship, and iHuman Virtual Patient Encounter hours. Documenting each encounter demonstrates cumulative practicum-related experiences in a single report. This report provides evidence of practicum experience and is required by state boards of nursing and the Commission on Collegiate Nursing Education (CCNE).

Course Outcomes

This assignment enables the student to meet the following course outcomes:

  • CO 1: Apply principles of nursing to the public health system. (POs 1, 6)
  • CO 2: Assess the health of populations, aggregates, and communities. (POs 3, 4)
  • CO 3: Apply the nursing process to vulnerable populations with professional clinical judgment and evidence-based practice. (POs 4, 8)
  • CO 4: Analyze the professional nursing roles in a variety of community settings. (PO 7)
  • CO 5: Utilize a collaborative approach to address factors that influence population health. (POs 2, 3)
  • CO 6: Examine collaborative trends in community health nursing. (POs 2, 5)

Directions

Complete the Practicum Log following the steps below.

  1. Download the NR436 Practicum LogLinks to an external .
  2. Each week, log the completed Practicum Project, Practicum Mentorship, and iHuman Virtual Patient Encounter hours. Save the log with each new entry.
  3. Record cumulative hours in the Week 2, 3, 4, and 6 Practicum Mentorship and Project Check-Ins.
  4. Complete, document, and submit at least 48 hours of practicum- related experiences by 11:59 p.m. MT on Wednesday of Week 8. Successful completion and documentation of 48 hours are required to pass the course. Do not submit the log until you have participated in the Week 8 Collaboration Café You may submit your log through Saturday, 11:59 pm MT, without penalty. Please note that submissions made after Saturday of Week 8, 11:59 pm MT, cannot be accepted.
  5. It is your responsibility to complete and document the required hours. Contact course faculty if you cannot complete and document the required hours.
  6. Abide by Chamberlain University’s academic integrity policy. Grading

Practicum Logs are required. Your log will be marked as complete (checkmark) if all criteria are met. If you receive an incomplete (X), you must notify your course faculty immediately. Practicum Logs must be marked complete to pass the course.
