Problem Statement – Version 1

A.W. is a 23-year-old female who presents to the clinic with a sore throat and a fever that began two days ago. She describes her sore throat as an “ice pick” in her throat and rates her pain an 11/10. She reported a sick contact earlier in the week and is not up to date with her flu vaccine. Her physical exam reveals that her tonsils are slightly enlarged, prominent exudates bilaterally, and erythematous posterior, with halitosis present. She also has tender bilateral cervical lymphadenopathy; the nodes are rubbery, mobile, and 1.5 cm. Her lung exam is clear from adventitious sounds.

Problem Statement – Version 2

A.W. is a 23 year-old college student with a history of infectious mononucleosis at age 15 who presents with acute pharyngitis and fever onset of symptoms 2 days ago. She reports tender cervical lymph nodes and an intermittent mild/dull headache. Patient exposed to roommate with similar sore throat symptoms. Examination reveals fever of 101.5 F, Posterior pharyngeal erythema, bilateral tender cervical lymphadenopathy with full cervical ROM and no nuchal rigidity. She is noted with Grade 3 enlarged tonsils and pronounced tonsillar exudate. Her uvula is midline. Negative for hepatosplenomegaly.

Problem Statement – Version 3

AW 23-year-old female with a history of mononucleosis, presents with a sore throat, fever, headache, and tender cervical lymphadenopathy. The physical exam provided a fever of 101.5 orally, posterior pharyngeal erythema, mild to moderate tonsillar enlargement without uvula deviation, tonsillar exudate, and tender bilateral anterior cervical lymphadenopathy without nuchal rigidity or hepatosplenomegaly.

Problem Statement – Version 4

A.W. is a generally healthy 23 YO female who presents with chief complaint of sore throat and associated headache, fever, and swollen neck glands for 2 days. States she is taking Tylenol for fever and eating ice cream to alleviate sore throat. The pain in the throat is 11/10 and “feels like an ice pic”.

Tonsils are mildly to moderately enlarge prominent exudates bilaterally Posterior pharyngeal erythema.
