Problem Statement – Version 1

Rebecca is a 23-year-old female presenting with a two-day history of sore throat, headache, fevers, and chills. On examination, she has a fever of 101.5, pharyngeal erythema and tonsillar exudate, swollen glands, and tender cervical lymphadenopathy. Rebecca reports recent exposure to a sick roommate who had similar symptoms. Rebecca is up to date on immunizations but has not received the annual flu vaccine.

Problem Statement – Version 2

Rebecca is a 23 year-old female law student with a history of infectious mononucleosis at age 15, presenting today with chief complaint of acute onset fever and sore throat x2 days. Associated symptoms include a mild headache, chills, and bilateral swollen neck glands. She reports that her roommate was also sick earlier this week, denies obtaining annual flu vaccination but did obtain COVID series. She has been taking acetaminophen for fever and eating ice cream to alleviate sore throat. She describes the sore throat like an “ice pick” with 10/10 pain resulting in difficulty swallowing. Physical exam reveals fever of 101.5, erythematous posterior pharynx, enlarged tonsils (right slightly larger than the left), prominent exudate, halitosis, and bilateral tender cervical lymphadenopathy but is negative for adventitious breath sounds.

Problem Statement – Version 3

R.F. is a 23-year-old female with a history of mononucleosis at age 15. She presents today with acute pharyngitis, associated fever, a headache, sore throat, difficulty swallowing, and tender cervical lymphadenopathy for the past 2 days. The physical exam is positive for fever of 101.5F orally, (+) chills, bilaterally mild to moderately enlarged tonsils without uvular deviation, prominent tonsillar exudate, posterior pharynx erythematous, tender bilateral anterior cervical lymphadenopathy without nuchal rigidity or hepatosplenomegaly and reporting pain 11/10. The patient reports positive exposure to a sick roommate earlier this week and has not had the annual flu vaccine otherwise up to date. She is in a monogamous relationship and occasionally performs oral sex.
