Harvey Hoya is a 57-year-old male who works as a flooring installer. He presents to the clinic for evaluation of an elevated blood pressure reading from a local health fair. The patient reports weight gain over the last year, resulting in a BMI of 28.8, and has a 35 pack-year smoking history. His past medical history includes gastritis and right ACL knee repair. Physical examination revealed blood pressure readings of 172/94 in the left arm and 178/98 in the right arm, with fundoscopic findings of AV nicking. There is a notable family history of hypertension and hypertension-related cardiac complications. The patient was previously informed that he has hypertension, but no action was taken at that time.

Harvey Hoya is a 57-year-old male that presents with elevated blood pressure. Pt is a tobacco smoker, overweight, and has poor eating habits. The physical exam is significant for HTN and AV nicking, indicating hypertensive retinopathy. Family history is significant for HTN, CAD, diabetes, and CVA.

H.H. is a 57-year-old Hispanic male working in construction installing doors who presents after a visit to a community health fair for a referral for high blood pressure. Denies headache, chest pain, or palpitations. Reported age- related weight gain with a BMI of 28.9 and knee pain associated with work. Physical exam is notable for Left Arm BP elevated at 174/96 and fundoscopic finding of AV nicking. FH consists of HTN and related complications. PMH includes being told he has HTN but does not follow up with a regular doctor.
Problem Statement – Version 4
Harvey Hoya is a 57 year old male construction worker who presents with chief complaint of high blood pressure found at a health fair, and has headaches in the morning with increased fatigue and 10 lb weight gain the last 6 months. Positive snoring while sleeping and difficulty sleeping. Positive family history of stroke, MI, and high blood pressure diagnosis. Patient states to smoke a pack of cigarettes a day since he was in his 20’s, daily coffee intake, daily fast food intake, and weekly alcohol intake. Limited daily exercise. Physical exam reveals severely elevated blood pressure, BMI of 28.7, waist of 45 inches. No SOB or chest pain. No known allergies and overdue for annual physical exam.
