In module 3, you will be submitting the population health assignment. Remember that the assignment is graded according to the rubric – be sure that all information the rubric requests is present. This paper is 3-5 pages and includes a minimum of 4 scholarly references with the last five years. Be sure to write your paper according to APA 7^th edition – I have attached the Paper template for this assignment.

2. Introduction – be sure to describe the role of assessment in population-focused health promotion, identify the selected geographical population (this is usually a county within a state) you plan to assess, and identify the sections in the paper. Use a minimum of one scholarly source in the introduction.
3. Population Health Assessment – be sure to describe the demographic “snapshot” of the population – include distribution of age, sex, ethnicity, marital status, and household type. Include morbidity and mortality findings as well as your findings related to assessment of the key social determinants impacting population health in the community (health and healthcare, social and community context, education, economic stability, neighborhood and built environment). There should be sufficient scholarly sources/empirical data – be sure that there is a citation for all information that is not common
4. Analysis of Findings – Provide a concise analysis of your assessment Identify any barriers to healthcare and key supports to healthcare within the region. Be sure to provide scholarly sources.
5. Recommendation – Identify one priority population health risk or disparity – validate with evidence from the population Recommend one population-focused solution to address the risk or disparity. Use scholarly sources to support the information.
6. Conclusion – Provide a conclusion summarizing the importance of the population health assessment and the impact of the proposed recommendation to address the identified risk or disparity. Describe the implications for advanced nursing practice.
