Problem Statement

M.Y. is a 69-year-old female with PMH of coronary artery disease, hypertension, hyperlipidemia, aortic stenosis, and high BMI. Chief complaint of intermittent chest pain for past 5 days. Pain is localized to chest. Symptoms worsen with mild exertion, episodes lasting less than 5 minutes. Associated symptoms include dyspnea and fatigue. Symptoms relieved by stopping activity and resting, resolving in 3-5 minutes. Denies fever, recent illness, nausea, and vomiting. Assessment reveals mild tachycardia, regular heart rhythm, and pre-existing grade II systolic murmur. Pain is not reproducible with patient change in position or chest palpation.
