Dylan Barton is a 42-year-old male who presents to the office due to frequent episodes of diarrhea that began 3 days ago accompanied by mid-abdominal pain. Patient describes the pain as achy, constant, and rates it as a 4/10. He states the pain gets as bad as 7-8/10 and crampy before he has diarrhea. States he has feel warm and flushed, and has been fatigued.

Patient has had nocturnal diarrhea. States he experiences episodes of small amounts of diarrhea every few hours, sometimes more than 6 times a day. Patient recently traveled to a conference in Chicago, denies exposure to anyone who was sick during this time. He states he did consume undercooked meat. Denies nausea, vomiting, chest pain, or shortness of breath. He has been able to tolerate oral intake, but has little appetite. Describes stool as watery, and he has had several bowel movements with bloody streaks.
