Sally Smith is a 34-year-old female with a history of Diabetes Type 1 and renal calculi (2 yrs ago) that comes in complaining of painful urination (9/10), burning-like frequency, dark urine (orange-yellow), decrease appetite, chills new onset, poor hyperglycemic control,(elevated bs) right achy flank pain (10/10). Upon physical examination patient presents with a fever of 101.6 F, HR 110, orthostatic hypotension (98/48), feeling light-headed when standing, and tenderness to the suprapubic area. The patient states has new male boyfriend/ sexual partner for 3 months, uses condoms as protection but not consistent, has normal menstrual cycles, allergic to penicillin.
