Scenario One
Mr. Johnson’s medical history includes hypertension, hyperlipidemia, and osteoarthritis.
He recently started treatment for a deep vein thrombosis. His current medications include warfarin, HCTZ, celecoxib, fluvastatin, and Goody’s Powder. Based on the information in this scenario, I would make a few changes. I would discontinue the Goody’s Powder because it has aspirin in it. The aspirin in Goody’s Powder can increase the risk of bleeding if taken with warfarin (Medscape, 2024a).

Scenario Two
Using the pill identifier and the characteristics of the medication given, this medication is propranolol 80 mg. Propranolol has many uses, including hypertension, migraine, angina, supraventricular arrhythmias, and pheochromocytoma (Medscape, n.d.-b). With this medication, providers should monitor heart rate and blood pressure because it can cause bradycardia or hypotension if the dose is too high for the patient.

Scenario Three
Alex had his labs drawn last week. He is a current smoker. His current medications include lisinopril, semaglutide, and St. John’s Wort. Alex’s recent lipid profile shows hypercholesteremia, hyperlipidemia, and hypertriglyceridemia. The treatment plan I would implement would be a moderate intensity statin and lifestyle changes, as recommended by the most recent clinical guideline by the JACC (Grundy et al., 2018).

Scenario Four
Sandra was recently prescribed Toprol XL for hypertension. She also takes venlafaxine for depression. Sandra stated she does not always take her blood pressure medication due to the side effects of fatigue and dizziness. Her blood pressure is elevated today. As her provider, I would educate Sandra on a few things. I would provide information about medication compliance, the risks versus benefits of not taking her medications as prescribed, and the fact that adherence to medications can improve overall outcomes (Aremu et al., 2022).
